What is Light Therapy?
Light Therapy is a procedure to treat Seasonal Affective Disorder, Sleep Disorders, and certain other conditions by exposing oneself to artificial light.
The artificial light source can be light boxes or light lamps producing a light intensity between 2,500 and 10,000 lux.
All living organisms have a biological clock that is influenced by the cycle of light and darkness.
This clock is a part of your brain and is located above the area where nerves travel to your eyes. This area is called Suprachiasmatic Nucleus (SCN).
What Are Circadian Rhythms?
As per the National Institute of General Medical Sciences, circadian rhythms are mental, physical, and behavioral changes that follow a 24-hour cycle. These natural processes respond to light and dark and impact all living beings.
How Do Circadian Rhythms Impact Your Health?
Your biological clock regulates your circadian rhythms including your sleep patterns, hormonal secretion, body metabolism, and feelings of alertness and sleepiness.
Thus, the timing of your body clock or circadian rhythms plays a significant role in the timing of your sleep period.
Now, there are some people who are more productive during the evenings, feel sleepy during the day, and have a tough time waking up in the morning. This means they have a delayed circadian rhythm.
However, there are others who feel alert during early mornings, feel sleepy early in the evenings, and wake up easily early in the morning. Thus, such individuals have an advanced circadian rhythm.
Accordingly, having a delayed or advanced body clock means a person is experiencing a sleep phase disorder.
Therefore, to adjust the body clock to its normal cycle, such a person needs to expose himself to a morning or bright light. And this process is called light therapy.
In light therapy, your eyes are exposed to intense but healthy light for specific repeated time intervals.
Artificial light in the form of light lamps is used in latitudes where sufficient light is not available, especially during the fall or the winter season.
Light Therapy for Depression
Light therapy is also used for treating Seasonal Affective Disorder (SAD). In fact, as per a study, Bright Light Therapy is also used to treat other depressive disorders.
These include chronic depression, premenstrual depression, antepartum depression, and bipolar depression.
Use of Light Therapy to Treat Depression
SCN
The Suprachiasmatic Nucleus (SCN)is the part of the human brain that incorporates the body clock regulating the specific rhythmic functions throughout the day.
It consists of thousands of neurons within the anterior hypothalamus that enable mammals to maintain the circadian cycle in their body functions.
However, this function of regulating circadian rhythm stops if the SCN is damaged.
Primarily, light regulates the circadian system within the human body. Thus, when light passes through your eyes, it falls on the retinal cells that perceive and convert the information regarding the light-dark cycle.
Further, these retinal cells are connected to the SCN through the Retinohypothalamic tract which is not a part of your visual system.
Remember that the regulation of your body clock or circadian system depends on the duration, wavelength, light intensity, and the time period for which your body is exposed to light.
Thus, the SCN is regulated by serotonergic neurons arising in the ralph nuclei. Also, the Pineal Gland gives direct feedback to the SCN.
The Pineal Gland gives feedback to the SCN through hormones called Melatonin and Serotonin.
Melatonin
Melatonin is a hormone secreted by the Pineal Gland in a specific circadian pattern depending on the light-dark cycle.
It is a chemical marker that signals seasonal variations of the light-dark cycle to all cells.
Further, Melatonin gets synthesized from its precursor Serotonin before it gets distributed in the body. And its pace of secretion is controlled by SCN.
Studies showcase that if a human is exposed to light, the circadian rhythms inside his body are altered. Further, it also suppresses the secretion of Melatonin.
Serotonin
Serotonin is another hormone that originates in the ralph nuclei of the brain stem. This hormone regulates your behavior, emotions, and circadian rhythms.
There are a number of studies showcasing that seasonal changes lead to change in the serotonin levels in the human brain.
Further, a change is also witnessed in serotonin-related behaviors.
The most important physiological mechanism that controls the synaptic serotonin levels is the reuptake of serotonin into the presynaptic neuron via serotonin transporter (5-HTT).
Remember that high 5-HTT density is associated with low extracellular serotonin and vice versa.
Thus, the highest 5-HTT density occurred during the dark months in autumn and winter. Whereas, the lowest 5-HTT density values occurred during the summer season.
This explains why SAD occurs during the winter or fall season in humans.
How Does Light Therapy Work?
As mentioned above, the Suprachiasmatic Nucleus (SCN) is a part of Hypothalamus, a peanut sized within the brain containing cells that regulate sleep and arousal.
The SCN receives information about light from the eyes when exposed to a light source and controls your behavioral rhythm.
Thus, individuals having a damaged SCN sleep throughout the day as they are unable to match their circadian rhythms with the light-dark cycle.
Further, the brainstem at the bottom of the brain passes neural signals to the hypothalamus to regulate the sleep-wake cycle.
You must know that the brain stem consists of structures like pons, medulla, and the midbrain. Also, the hypothalamus and the brain contain cells that promote sleep and hence generate a brain chemical called GABA.
This chemical is responsible for reducing the activity of the arousal centers within the hypothalamus and the brain stem. It is important to note that even the brainstem sends signals that relax your body.
Now, the Pineal gland located between the two hemispheres of the brain receives signals from the SCN.
These signals enhance the production of melatonin – a sleep inducing hormone. Thus, the increased production of melatonin helps you sleep once the lights go down.
However, some of you are unable to coordinate their natural sleep-wake cycle using natural light. Hence you need to take some amount of melatonin at the same time during each day in order to stabilize your sleep patterns.
As mentioned above, when artificial light falls on the melanopsin – the receptor cells in the retinal ganglion – it converts light into neural signals.
Remember that melanopsin in the SCN is responsible for suppressing pineal melatonin secretion.
Further, it also improves alertness and decreases depression.

Do Light Therapy Lamps Work?
Bright Light Therapy is a non-pharmacological treatment that is used to treat SAD.
The American Psychiatric Association requested to carry out a number of independent studies to test the efficacy of BLT in seasonal and non-seasonal depression.
In all the trials, dawn simulation was used to test for SAD and BLT was used as an ancillary therapy to psychopharmacological treatment in non-seasonal depression.
The results of such research showcased impressive results. In fact, the results were either equal or superior to the results displayed by most psychopharmacological trials.
However, none of the results were in favor of the BLT ancillary to psychopharmacological treatment in non-seasonal depression.
In addition to this, a subsequent placebo-controlled randomized trial of BLT showcased results in favor of light therapy.
This trial was carried out in a sample of 102 patients with non-seasonal depression who were treated with sertraline.
The study showcased that patients exposed to bright light experienced a reduction in depression.
Another study was conducted among randomly selected people from four areas: Nashua, New York, Montgomery County, and Sarasota. These individuals were sent a Seasonal Pattern Assessment Questionnaire (SPAQ) to estimate the prevalence rates of winter SAD, summer SAD, and subsyndromal winter SAD.
The results of the study showcased that the Northern Latitudes showcased a higher rate of winter SAD and subsyndromal SAD.
In addition to this, the study showcased a positive relationship between the latitude and a higher rate of winter SAD prevalence in the age groups over 35 years.
Further, no correlation existed between the latitude and summer SAD.
In addition to this, another study was conducted to study the impact of latitude on the prevalence of seasonal depression.
In this study, random telephone numbers of individuals across 8 degrees of latitude for the province of Ontario were collected.
In other words, an equal sample of people was taken from 8 strata of 1 degree each and who became a part of the experiment for a 12-month period.
As a part of the experiment, each individual over 20 years of age and having lived in the region for 3 years or more was interviewed. It was a structured diagnostic interview that was undertaken over a telephone call.
In all, 1605 individuals completed the interview. Further, the results of such an experiment showcased that the prevalence of lifetime SAD was 2.6%.
However, there was no impact of the latitude on the prevalence of either major depression or the seasonal subtype across the 8 strata.
But, the latitude was observed to be negatively correlated with the severity of the seasonal change in mood of an individual.
How to Use a Light Therapy Lamp?
First, you need a light therapy lamp that is clinically tested to treat depressive disorders and sleep disorders.
Further, such a therapy light must have an intensity of 10,000 LUX. In case it is white light, then the intensity must be 3,000 – 4,000 Kelvin.
Additionally, the light must have a broad field of illumination and be UV-filtered. Also, the therapy lamps must incorporate a glare-free diffuser and should be adjustable for optimal positioning.
To use the light therapy boxes or lamps, you first need to place the lamp at the center in front of your eyes.
Then, you need to set the lamp enclosure at about a 30-degree angle from its vertical height. Besides this, the middle of the lamp enclosure must be 12 to 14 inches away from your eyes.
